How to get a replacement Total Mobility AT HOP card
Step 1 - Phone your Total Mobility asssessor agency
You will need to phone your Total Mobility assessor agency and advise them your card is lost or stolen.
You don't need to send a new photo, but you will be charged for the replacement card.
Step 2 - Payment
The charge for the replacement card is $10 and must be paid to the agency.
Step 3 - Dispatch of the replacement card
It can take approximately 30 days after your request to receive your replacement card.
Please note, you will be charged full fares until the card is replaced.
